Service Description

Consultations are complementary. Shortly after you schedule, you will receive an invitation to join a secure patient portal called Charm Health. You will create an account by following the link that will be sent to your email. The portal contains a consent to treatment that is required to be completed 24 hours prior to your appointment, or the appointment may be cancelled. If you experience any issues with registering on the portal, you may contact Angela at 717-332-4774 and she will assist you. If you book before 24 hours prior to your appointment, the questionnaire may be completed up to an hour prior to your appointment. The consent to treatment is required, and the medical questionnaire is appreciated, but not required. O
